Attendance/Tardy Form

TO BE FILLED OUT BY PARENT/GUARDIAN ONLY: This form needs to be filled out each and every time your student is absent. It must be completed by 9 AM. All Early Dismissals are to be called into the Front Office 941-371-4917 x110 between 7am-9am. Please include the PARENT email address on file with CMHS in Renweb / FACTS SIS for verification. PROVIDE A PARENT CELL NUMBER THAT MAYBE CALLED FOR VERIFICATION.
